Geriatric Care
Geriatric medicine is a medical specialty that is related to the specific health care needs of older adults.
The Indian elderly face several social issues such as loneliness, elder abuse, neglect, lack of income security, and poor access to health care. According to the National Council on Aging, about 92 percent of seniors have at least one chronic disease and 77 percent have at least two. Heart disease, stroke, cancer, and diabetes are among the most common.
Since 2012 we have been providing geriatric care services to the underprivileged elderly in our community. Our geriatric care services are availed by about 1200 persons every month.
There are many conditions that affect people as they grow older and include but are not limited to the following: Arthritis, Osteoporosis, Cancer, Alzheimer’s Disease, Hip and Joint Replacement, Balance Disorders Incontinence, Cardiovascular Diseases like Heart Disease, Stroke etc.
Some of the key issues that highlight the necessity of Geriatric care include: Non-availablity of hospital care, financially dependent on childdr and relatives, failing health, isolation, lack of preparedness, transportation issues, absence sociaal security, difficulty in getting cotinuum of care, lack of proper referral mechanism and difficulty in doing day to day activities caused by disabilities in old age.
Out-patient Clinics
We conduct out-patient clinics at different locations in the city to reach out to the elderly in the community, throughout the year, on designated days of the week. Similar geriatric care clinics are conducted in the rural and tribal areas. All the elderly and the needy are screened and treated for all ailments concerned. A medical team specialized in geriatric care, screen and treat for all ailments with special attention to hypertension and diabetes. Free medicines and health check ups are provided

Old Age Home Visits
Our medical teams along with committed volunteers sensitized and trained in basics of geriatric care, visit various old age homes in the city regularly and provide necessary health care and medicines free of cost.
Respite Care
Caregiving for an elderly, ill, or disabled family member is a demanding job and no one is equipped to do it alone. Respite care provides temporary relief for a primary caregiver, enabling them to take a much-needed break from the demands of caregiving.Whether it’s for just a few days or few weeks, seeking respite care can help ease the burden of family caregiving and can protect them from becoming exhausted, isolated, or even burned out.

Transitional Care
Transitional Care refers to the continuity of health care of patients during their movement from a hospital to their homes. Transitional care can help the patients improve their ability to manage their daily routine once they go back home. It can benefit persons recovering from long hospital stay due to illness, injury or surgery.
Age Care Founation’s up-coming center for integrated Geriatric and Palliative care wll have the necessary multi disciplinary rehabilitation care team that will collaborate to help in overall patient care for a holistic recovery, which includes physical recuperation as well as achieving emotional stability and quality of life.
Rehabilitative Care
Geriatric Rehabilitation aims to restore function or enhance residual functional capability and improve the quality of life in older people i.e. particularly those with disabling impairments and/or frailty. Goals focus on returning patients back to the physical and mental functioning they were at before the health episode occurred

Long Term Care
Long term care is the broad range of personal, social and medical services that assist people who have functional or cognitive limitations in thier ability to perform self-care and other activities. People often need long-term care when they have a serious, ongoing health condition or disability. The need for long-term care can arise suddenly, such as after a heart attack or stroke. Most often, however, it develops gradually, as people get older and frailer or as an illness or disability gets worse.
Basically, any patients who may be unable to function on their own at home safely and independently can be helped by long term care – whether this is a temporary situation, with that person needing help for a few weeks before regaining their independence or a permanent one that creates a need for ongoing help.
Dementia Care
Dementia is a collection of symptoms which affects mental processes in a number of ways, from impaired communication to memory loss. There are many underlying causes of dementia, including Alzheimer’s disease. Dementia care is a specialist support system which is designed to support those living with dementia – it is provided by carers who are experienced and trained in caring for someone with dementia.
Age Care Foundation is actvely engaged in creating public awareness about dementia and in close association with ARDSI – Visakhapatnam (Alzhiemer’s and Related Disorders Society of India) working to bring about a dementia friendly community where those affected by dementia will have better quality of life.
