About Us
The beginning….
Sneha Sandhya, a group of senior citizens with a zeal to serve the elderly, was established in the year 2004. They realised that accessible and affordable healthcare is not available for most of the elderly in the community.
This prompted them to promote an organisation dedicated to provide Geriatric Care to elders and also Palliative Care to persons with serious illnesses. Thus was born “Sneha Sandhya Age Care Foundation” registered as a Public Charitable Trust, in 2012.
Our Mission
Provide specialized health care services for elderly and also terminally ill persons, in all health care settings on a continuous basis. That is home, clinic, hospital or hospice.
Our Vision
To become a center of excellence for two medical subspecialties, Geriatric and Palliative care.
What we do
We have a community participatory approach to our work, involving a large group of committed volunteers, sensitized and trained in palliative/geriatric/dementia care practices.
We organize outreach services to the elderly in the community, old age homes and the terminally sick people especially those suffering from cancer, right at their door step.
We sensitize, inspire and empower individuals and organizations to increase access and optimize the practice of palliative care and dementia care.
We build capacity through education and training; promote similar services across the state through awareness creation, advocacy and partnerships with government, private health sector and other non governmental organizations working in the areas of geriatric/palliative/dementia care.

THE NEW HORIZON
We are in the process of creating a ‘hub and spoke’ model with a central facility at Visakhapatnam, and satellite centers in the districts of Srikakulam and Vizianagaram. The central facility will provide wide range of specialized health services which include Palliative care, Geriatric care, Dementia care, Long term care, Rehabilitative care and Respite care whereas the satellite centers will provide pain relief and palliative care.

OUR UPCOMING INTEGRATED
CENTER FOR GERIATRIC & PALLIATIVE CARE
The year 2022 is a landmark year in the journey of Age Care Foundation, as Free Polio Surgcal & Research Foundation has provided one acre of land in Visakhapatnam to establish and run a fully integrated center for Geriatric & Palliative care, the first of its kind in Andhra Pradesh. This upcoming project will open new vistas of care and scale up Age Care Foundation’s services manifold.
The center along with its out-reach services is projected to serve people not only from the city of Visakhapatnam but also from the adjoining districts. The hospital is aimed at serving the unmet needs of the community and will be run on a not for profit basis. Age Care Foundation’s existing network of doctors, nuses, volunteers and social workers will continue to serve the community frm the new facility.
